A Case of Multiple Myeloma with Sclerosis and Cryoglobulinemia

Abstract
Multiple myeloma is a malignant disease that results from the neoplastic proliferation
of a clone of cells with features of B-lymphocytes. The clinical manifestations vary but
include tumor formation, osteolysis, hemopoiesis, hypergammaglobulinemia, renal disease
and paraproteinemia. Osteosclerotic myeloma is a very rare lesion, found in an estimated
3 percent of all cases of myeloma.
The authors experienced one case of multiple myeloma with sclerosis and
cryoglobulinemia.
The 64-year-old male patient was admitted to our hospital because of tingling sense
and pain in his hands and feet. He appeared chronically ill and there were discrete
purpuric patches on lower extrimeties especially below the knee.
Hypergammaglobulinemia was noted and IgG Kappa type was identified by
immunoelectrophoresis. A test for cryoglobulinemia was positive. Radio-logically, the
skull series showed irregular ill defined sclerotic changes on the left frontoparietal
region. The bone marrow biopsy performed on the left posterior iliac crest revealed
myelomatous infiltration.
The authors report this case with review of literatures.
